- Psychiatric Medications and Care in Mesa AZ: Meet Dr. Meghan Orbegoso, DNP, PMHNP-BCMeet Meghan Orbegoso, MSN, PMHNP-BC, a compassionate Psychiatric Nurse Practitioner offering expert care and psychiatric medications in Tempe, AZ. Dr. Meghan Orbegoso, DNP, PMHNP-BC Hi! I’m Meghan, but feel free to call me Meg or Dr. O. I am a psychiatric nurse and nurse educator with many years’ experience in most mental health settings including outpatient, voluntary inpatient, court ordered inpatient, SMI populations, psychiatric urgent care and crisis units. I work at Phoenix Children’s inpatient psych unit and float to the behavioral health area in the emergency department. I also teach mental health specialty for nursing students. I chose to go into education and become a nurse practitioner because I am determined to be the change I want to see in the world. During my nursing education and nurse practitioner education journey, there were no instructors that looked like me or looked like anyone that I grew up with or that I felt could relate to BIPOC, first-generation, or even to barriers of social determinants of health. My family background is Peruvian and my husband is Dominican and we love to EAT! We love trying new dishes and cuisines we have not tried yet so please share any recommendations! Our toddler is still in his chicken nuggets only phase, but he will hopefully see the light and seasoning by his ancestors soon. When I’m not working or spending time with family, I like to dissociate in light-hearted media including YouTube, mobile games, fan-fictions, anime, and BBC’s 1995 Pride and Prejudice. I stay informed and therefore stay away from fictional media that is serious; the real-life state of the world is drama enough! Child/Adolescent With a focus on child and adolescent mental health, Meg is dedicated to helping young people and their families navigate challenges with confidence and understanding. She provides clear, practical guidance tailored to each child’s developmental stage, ensuring both emotional well-being and safety. Her supportive approach fosters resilience, open communication, and a strong foundation. Neurodivergence While Meg is not yet trained in diagnosing Autism, she is phenomenal at offering neuroaffirming care. She takes time to learn each person’s unique strengths, challenges, sensory needs and communication styles. Anxiety & Mood Meg offers a comforting presence and the ability to help navigate anxiety, depression, irritability, and pediatric bipolar disorder. Her therapeutic style blends evidence-based strategies with compassionate listening and empathy. Families and children alike appreciate her calm demeanor and commitment to creating a safe, supportive environment for healing and growth. Q&A What is your favorite movie? Lion King Favorite superhero? Class C Hero - Saitama! If you could have a superpower, what superpower would you pick? To speak every language and always have the right words to say. If you had 3 wishes, what would you wish for? That everyone had access to mental health care, a solid support system, and a loving community. That our days and weeks and months were longer and not to get more done, but to rest, spend time, and enjoy. For everyone to feel safe, to feel seen and known, to feel they are enough, and to believe they deserve care and kindness. Tell us a fun fact about yourself: I have serious stage-fright. I don't know how I decided to become an educator and lead lectures/lessons for 50+ nursing students at a time. I know why I want to do it, but I still don't get how I manage to speak coherently while I internally panic. Tell us about your pets: I have a Pit Mix named Goose. He got his name from his honk-like kennel cough he had when we rescued him. He is a good boy What are ways beyond medication that can improve your mental health? All of the self-care activities! All of the positive lifestyle practices that make sense for you and your family and community. Connecting with nature, caring for yourself emotionally, spiritually, holistically matters. Having strong healthy relationships matter! The results from the 80+ year Harvard wellbeing study are clear: "The clearest message that we get from this 75-year study is this: Good relationships keep us happier and healthier. Period." - R. Waldinger, Director of the Harvard Study. If a patient is nervous about an evaluation, especially when talking about tough things like trauma or sexual abuse, what would you want them to know? You are safe with me. We can discuss as little or as much as you feel ready, willing, and able to share in the moment. I will not push you to disclose more than you want to. What is your philosophy on using medication to treat mental health? Are medications lifelong? My favorite medication is Therapy. It has the fewest side effects and the longest half-life. :) But really, I believe medication can be a valuable resource and tool in a client's treatment plan, but should not be considered in isolation. I want to work with you and collaborate on all aspects of mental health which includes medications and more. Whether medications are life-long will vary from client to client and from situation to situation. What are your thoughts about treating adult ADHD? I was diagnosed with ADHD in adulthood. I grew up in a family that did not believe in mental health. That struggle with executive function was just laziness and the only treatment was to "ponte las pilas" or "work harder". I had teachers from kindergarten to college say "she's smart, but she would do better if she just applied herself." I failed out two Universities and several classes in my life until I learned about mental health in nursing school. The day after the first lecture on mental health and illness I scheduled a psychiatric evaluation. I was expecting a diagnosis of anxiety and ended up learning I was depressed and struggled with ADHD. I started treatment and the world opened up. I went from failing and being withdrawn due to academic performance to graduating with 3.8 for my Bachelors and Doctorate degrees. I have a lot of thoughts and feelings when it comes to adult ADHD. A wonderful psychiatric nurse practitioner diagnosed me and still manages my treatment and I look forward to helping my future clients in their journey. Yes, I shared a bit about my experience with adult ADHD in the previous question. In that first psychiatric evaluation, I learned I also had been struggling with depression. I remember being told that it was not normal to constantly feel like a burden and having negative thoughts playing on a loop in your mind. I was shooketh! And let me tell you when I started my first antidepressant and the negative thoughts slowed and eventually stopped! Flabber-fucking-ghasted! Because what do you mean I just needed store bought serotonin to stop feeling like a constant burden!? Witch-craft! Brujeria! And I love it! I want to help others also realize they are worthy of taking up space! I want you to man-spread to the heavens (figuratively of course, but also you do you!) with how comfortable you are with taking up space and knowing you deserve care, you deserve kindness, and you deserve to feel and know that you are enough just as you are :)

- Psychiatric Medications and Care in Phoenix AZ: Meet Dr. Azuka, CNM, DNP, PMHNPMeet Dr. Azuka Okafo, CNM, PMHNP-BC, DNP, a compassionate Psychiatric Nurse Practitioner offering telehealth expert care and psychiatric medications. Dr. Azuka Okafo, CNM, PMHNP-BC, DNP I am double boarded as a nurse midwife and a psychiatric nurse practitioner. I have been a practicing nurse midwife for over two decades and decided to add formal psychiatric training to support my passion for holistic women’s healthcare. I have worked in diverse settings including a private practice in suburban Connecticut, urban health in Baltimore, Maryland, and most recently at a tribal hospital in rural Arizona. My training and interests extend beyond reproductive psychiatry and include SMI and pediatric mental health. I grew up in Africa and have a deep appreciation for cultural influences on health. I am also acutely aware of the impact of mental health on overall well-being. My work on the reservation highlighted the need for a bridge between reproductive and mental health care. I earned Postpartum Support International’s Perinatal Mental Health Certification in 2024 while enrolled in my PMNHP training program. My prescribing philosophy is that medications are one of many options available to support clients. Every person’s needs are unique, and I firmly believe in collaborating with clients to create holistic treatment plans. Being a full-scope nurse midwife did not provide me with much free time, but I enjoy catching up with good friends over tea or a meal, taking walks, reading, working out, watching sparring hummingbirds, and meeting neighborhood dogs. I’d say my vibe is energetic, curious, humorous, compassionate, and collaborative — with a dash of sarcasm and low tolerance for nonsense. I take my work seriously but never myself. Perinatal Care Azuka has spent the last chapter of her career on the Rez delivering babies. As a certified nurse midwife, she has a strong understanding of the intersection of complex pregnancies and mental health. Hormones While many psychiatric providers claim they understand the impact of hormones on mental health, Azuka *truly* understands and has the clinical experience to back it. Adolescents She's really good with teenagers, and can definitely have a fun, silly vibe. If you needed a person who was going to bring some spirit into appointments, Azuka's here for it. Q&A What is your favorite music? I don’t have a favorite type per se, because I grew up in a family where we listened to various genres (classical, jazz, folk, reggae, rock, zouk, R&B, Latin, soul, and funk, etc.) I do have a deep appreciation for classical/Spanish guitar music. Favorite superhero? Batman. He became a superhero as a response to his childhood trauma; I appreciate the complexity of his character. If you could have a superpower, what superpower would you pick? I think that it would be cool to control the weather. I’m looking at you, Storm. Tell me about your pets. I lost my beloved German Shepard/Chihuahua mix in 2024. That dog was the quirkiest animal that I didn’t know I needed in my life. I considered myself to be her therapy human, as frequent walks, pets, dietary changes, and love helped her blossom from an anxious dog into a social pup. I became her medication manager in her twilight years when she required two different medications to ease her escalating anxiety. Fun fact about you: I once caught a baby in my soccer uniform—without the cleats. What are some experiences that make you unique? I have been a midwife on both ends of life; at 14 I held my father’s hand as he left this world. What are ways beyond medication that can improve your mental health? Medication is simply a piece of the whole puzzle and often work better when combined with therapy. I am a firm believer that social support, diet, exercise, and sleep are critical to maintaining one’s mental health. I have been sleep deprived for decades…the need for sleep is real! Are medications lifelong? Does a patient who sees you HAVE to be on medication? No, someone who sees me does not have to be on medication. There are some instances, however, where a person may need to be on medication long term because of that individual’s unique circumstances. If a patient is nervous about an evaluation, especially when talking about tough things like trauma or sexual abuse, what would you want them to know? I would want them to know that their safety is my main concern and the evaluation an information-gathering session so that I can understand their story. We will only talk about the hard things if they are comfortable doing so. Do you have any personal mental health experiences you can share with us? I worked at a hospital on a reservation during the COVID-19 pandemic. We were isolated and scared; the sound of multiple helicopters used to transport critically ill patients was deeply disturbing. I still brought love and levity to patients birthing amidst the chaos while concealing my own anxiety. We were isolated from our friends and families by fear and local curfews. My self-care suffered and led to a devastating injury and burnout. But I’m also a Black woman so…

Midnight in Paris, confession- I’m a Francophile. I love anything and everything that has to do with anything French and history, which is the perfect combination for this movie in which the main character (Owen Wilson) time travels to the 1920’s, the Golden Age in Paris and mingles with all the cultural greats including the Salvador Dali, Picasso, Ernest Hemingway, etc. I also love the book The Nightingale by Kristin Hannah, and there are rumors that there will be a movie adaptation, so adding that to the future favorites list as well. Favorite superhero? Mulan! Growing up, she was the first “superhero” that I could relate to who looked like me as a young teenager and it was so empowering. Add to that, the fact that she was training with the boys and constantly enduring obstacles and odds that were against her as a female warrior in a male dominated world? Amazing! If you could have a superpower, what superpower would you pick? Definitely teleport to any point in history. I would love to live a day in different time periods anywhere in the world. Be a shopkeeper in ancient Rome, Parisian florist in the 1940s, an activist in the American 70’s. Favorite bands/singer? ome of my favorites include Lady Gaga (best performance I have ever seen live), Daft Punk, Glass Animals, Masego, Jungle, Tems, Post Malone, Kehlani, Beyonce, J.Cole, and I enjoy frequenting international playlists on spotify and seeing what is popular elsewhere. If you had 3 wishes, what would you wish for? A radical transformation in the hearts and minds of humanity, where everyone can live in peace and solidarity without division and hate. Eradication of politics, and creation of a magical system that supports the best interest of every human being. Free airline travel for everyone to anywhere. Tell us a fun fact about yourself: I am a huge sports fan! I love mainly basketball, baseball, football, and the Olympics, and you can catch me tearing up throughout the entire opening and closing ceremonies. Go Suns, Cards, Dbacks! Tell us about your pets: I have one dog her name is Salem, she’s a Maltese-Poodle mix who is an introvert and lives comfortably with her anxiety. She will only drink water if you pretend to dump out the old and put new water in her bowl, likes her alone time, but is the sweetest, most well-behaved little fur-child. What are other things you feel could be beneficial to improve mental health other than medications?: I mentioned some in my previous answer, but there are so many aspects to improving our mental health other than medications, and I absolutely love diving into these conversations. I am a reformed workout junkie, believe in the idea of exercise in strengthening our mental health whether it starts with taking brief walks throughout the day or strength training. If a patient is nervous about an evaluation, especially when talking about tough things like trauma or sexual abuse, what would you want them to know? I may repeat this when we meet in real life, but I want you to know that I am here for you. This is your time, your story, you are in control, and I will provide you with a safe, confidential, and non-judgmental space. It’s difficult enough to have to repeat these difficult topics, but I am here to actively support you though your journey. You've mentioned being passionate about working with people with religious/cultural trauma - can you tell us about that? Many people who endure any form of trauma due to either religion or culture most often do not have a choice in which group or setting they were born, unfortunately we don’t get to choose what kind of life we will live. I distinctly remember growing up wondering why I was born with certain contexts in place. Religions and cultures have many characteristics, teachings, definitions, and beliefs that can provide positive guidance and wonderful benefits in life, but unfortunately much stigma, a life of surveillance, prejudice, lack of acceptance, and discrimination can also arise. Working with those who have shared these experiences is important to me because these forms of trauma can deeply shape a person’s identity, relationships, and worldview, creating long-lasting effects on mental health. I am here for you to rediscover your own agency and feel validated in your experience. Do you have any personal experiences regarding mental health you are open to sharing? sharing? After I had my first baby I was very confused and scared by high anxiety and intrusive thoughts. Even being in the field for so many years I had never even heard of postpartum OCD. Now every new parent I run into I'm like "How are you REALLY doing?" and it has made me very eager to support other parents! What is your philosophy on using medication to treat mental health? Are medications lifelong? Does a patient who sees you HAVE to be on medication? Whether or not someone wants to be on medications is completely up to them. There are many different factors that may play a role in the decision, but ultimately medications are only a piece of the larger puzzle, and sometimes to complete the puzzle, they may need to be utilized depending on the situation. I do believe in holistic and functional psychiatry as well, involving various components such as nutritional interventions, gut brain health, hormones, lifestyle factors to name a few, but sometimes we need help with the neurotransmitters in our brain that may be imbalanced and need some encouragement to get back on track. It is also impossible to say if medications need to be lifelong, because just like that, life is long and it truly depends on the multiple mile markers along your journey. Do you have any personal experiences regarding mental health you are open to sharing? When I came out to my Korean parents at age 15, they called a Catholic priest and psychiatrist (both Korean) over to our house. I’m sure you can tell what kind of ride I’ve had in my own mental health experience. Part of the reason why I immediately wanted to study psychology in college was so I could help another young girl with so much angst understand why they were feeling this way, and that it was going to get better. Guess what, it did get better, and now I have the pleasure of pursuing my lifelong goal! How do you feel about marijuana? I completely support the powerful and positive benefits of marijuana, but with anything in life, in moderation and depending on your specific situation. What are your thoughts about treating adult ADHD? I was never treated for ADHD as a child because my parents did not believe in mental health care, but I remember struggling so much throughout my younger life with all the symptoms but thought it was normal. I didn’t realize until I was older that I was working much harder than others to accomplish the same tasks. Since then, I have been treated for ADHD as an adult, and firmly believe that proper assessment and treatment can be a gamechanger for many who continue to struggle in their adult years.
