Please note that we require a credit card on file for copays, deductibles, and self pay fees.
Please read the policy section and the consent upon booking regarding refunds, contacting your provider no-shows and cancellations. If you cancel less than 24 hours prior to appointment, miss your appointment or are late by over 10 minutes, you are responsible for the full self-pay fee, regardless of insurance status. Please ensure you cancel on time. We cannot make exceptions.

Psychiatric Evaluations
$300+
A psychiatric evaluation is a 60-90 minute appointment in which we will ask a lot of questions about sleep, your mood, medications, and so on! We can discuss medication and supplement options, diagnosis, and other recommendations. We do child/adolescent evaluations, adult evaluations, family planning/peripartum evaluations, postpartum evaluations, pre-operation evaluations, asylum evaluations, ESA evaluations + more!

Follow Ups + Consultations
$120+
Follow ups are a 15-30 minute appointment in which you can discuss how you are doing, your sleep, appetite, medications, and ongoing things in your life. We can discuss medication adjustments or changes, review labs, or even just check in to see if you're doing okay! We also offer initial 30 minute *finding the right fit* visits for the same price.
If you are a parent who wants a visit without their child present, please note this will always be self pay. Insurance will not cover unless your child is present.

Neurodivergence Evaluations
$600 to $1000
Evaluations for ADHD + Autism Spectrum Disorder by experienced nurse practitioners. It includes a clinical interview, TOVA/Qbtest, MIFGAS testing or the ADOS, and a thirty minute follow up to review results.
**SELF-PAY ONLY!!! THESE ARE NOT COVERED BY INSURANCE AND CAN NOT BE REIMBURSED VIA SUPERBILL.**

QB Tests & Tova Tests
$150
Computerized ADHD testing available at Flagstaff, Tempe, Tucson & North PHX locations. Each test comes with a report.

Please note, this is not diagnostic and we do not offer interpretation without a full evaluation if you are not an existing patient.

Paperwork + Records
$25+
We can complete paperwork at your provider's discretion for employment, accommodations, educational concerns, emotional support animal letters, disability, surgeries, trans health, FMLA, and more as needed.
For a simple document, it starts at $25. We charge an additional $25 per 15 minutes to complete complex paperwork.
All paperwork requires 72 business hours to complete. If you would like the form same day, it will be a $100 fee for expediting. Medical record requests are $50 per request.

Genetic Testing
Per Insurance
We offer services from Genomind and Genesight. We will send a kit to your house to swab you (or your child's cheek). We will receive a report of how you metabolize medications and if you have the MTHFR gene mutation. We do require a 30 minute follow up to review the results.

Ketamine Evaluation
$250/60 minutes
We offer Ketamine Injectables in conjunction with Find Your Shine therapy for Ketamine Assisted Psychotherapy. We also provide guidance if you would like to use a guide within Paperflower.
Evaluations are required whether you are an existing or new patient. You may be able to have a superbill for some reimbursement from your insurance.
These evaluations offer medical and psychiatric clearance to receive Ketamine within our clinics for therapy.

Ketamine Administration with Find Your Shine
$300/injection
Guided Injection with Paperflower
$500 per session
All Ketamine Administration is currently done at our Tempe office and in collaboration with a guide -- either John or Find Your Shine Therapist for Ketamine Assisted Psychotherapy. This does not encompass the therapy fee.
Please contact your therapist at Find Your Shine to coordinate a time for your injection.

If you would like to use John as a guide for your Ketamine journey through Paperflower, we can offer a package with an evaluation & initial virtual prep session with your guide.
Evaluations are required whether you are an existing or new patient, and are not covered by insurance. You may be able to use a superbill for some reimbursement.
Your prep session with your guide will review your intentions, and provide guidance on what to expect on your journey. Integration sessions can be booked after for $50 per session.

After Hours Calls
$100/30 minutes
Emergency after-hour calls are typically not taken as we are not an emergency service. However, if your provider decides to offer an after-hours call for your specific situation, it will be a fee of $100/30 minutes which is not covered by insurance. Please be aware of resources such as Mind 24/7 and the Crisis Team which may be more appropriate.
Same Day Refills
$10 to $15
We ask for 72 business hours to complete refill requests. If you run out of pills and need an urgent same-day request, there is a $10 processing fee. For after hour same-day requests, weekends or holidays, there is a $15 processing fee.
